What's inside

  • Dog profile, owner details, vet and emergency contacts
  • Logs: vaccinations, flea/tick/worm treatments, vet visits, medication with 31-day chart
  • Monthly weight and body condition, grooming
  • 12-month care calendar and a 'when to call the vet' checklist

Why keep a dog health record?

When the vet asks when the last worm treatment was, how much your dog weighed in spring or when the itching started, the answers are in one folder. A dog profile with microchip, insurance and allergies, owner details, vet and emergency contacts and a daily routine page also make it easy to hand over to a sitter.

Vaccination record

25 rows for the date given, what the vaccine protects against, batch or sticker, vet or clinic and the next due date. Copy the details from your dog's vaccination card or vet records; your vet decides which vaccines your dog needs and when.

Flea, tick and worm treatment log

25 rows for the date, type of treatment, product, your dog's weight and the next due date, so nothing is missed or given twice. Use the product your vet recommends and follow the label or your vet's instructions.

Vet visit log

11 large rows for the clinic, the reason for the visit, findings and advice, and the follow-up, with room to jot down your questions before an appointment.

Medication log with daily tick chart

Record any medication your vet prescribes, with the instructions exactly as given, plus a 31-day morning and evening tick chart. The log is for keeping records only and never replaces your vet's instructions.

Weight and body condition tracker

A monthly tracker from January to December for weight, body condition on your vet's scale and notes, plus a short body condition guide to go through with your vet. Slow changes are easy to miss without a record.

Grooming and care log

25 rows to tick off brushing, baths, nails, teeth, ears, eyes and paws, and to note anything unusual such as lumps, sore spots or ticks.

A year of care: the 12-month calendar

A 12-month care calendar for weigh-ins, parasite care, nails, teeth, coat and skin, plus a once-a-year list: health check, vaccinations as advised by your vet, microchip details, insurance, ID tag and leash, and travel papers in good time before holidays.

Symptom notes and when to call the vet

Symptom and observation notes for what you noticed, when it started and how often it happens, and a general "when to call the vet" checklist in three parts: straight away, soon (the same or next day) and what to have ready before you call. It is general guidance only and never replaces your vet.

FAQ

What should a dog health record include?

The dog's profile and microchip, your vet and emergency contacts, vaccination dates, flea, tick and worm treatments, vet visits, any medication, weight over time and grooming. This planner has a page for each, plus a 12-month care calendar.

Does it replace my dog's vaccination card or pet passport?

No. Keep the official papers from your vet. This is your own home record, so you have every date and note in one place, and you can copy the dates from the official papers.

Does it tell me when to vaccinate or which medicine to give?

No. It contains no treatments, doses or schedules. Write down what your vet tells you or what is on the label. The "when to call the vet" page is general guidance and never replaces your vet.

Can I print extra pages?

Yes. It is a printable PDF, and the planner suggests printing extra copies of the logs you use most, such as parasite treatments, weight and grooming.

What's included?

A 14-page printable A4 PDF in English, Norwegian, Danish, Swedish and German, downloaded right after purchase. Nothing is shipped.
